"Data service is unavailable" Errors when Starting Insight Applications

Question / Problem: 

This error is displayed when attempting to launch any Insight application. What is the cause and how can it be resolved?

Answer / Solution: 

When enabling Windows authentication on your IIS server(s) to support Insight using Windows authentication scheme, it is required to leave Anonymous access for the data service ("WcfDataService") application enabled - simply enable Windows authentication in addition to that.

This requirement exists for all supported versions of Insight.
