MarkView Viewer Enterprise & Express - "OK" button not visible on Re-Create Document markup in MarkView 6.5

Behavior
- Following an upgrade from MarkView 6.4 to MarkView 6.5, the "OK" button is not visible on the Re-Create Document markup.
Known Causes
- This issue can occur following an upgrade from MarkView 6.4
Resolution
- The issue which can be resolved by taking the following steps.

Connect to the MarkView schema via SQL*Plus

Run the attached SQL Fix36750.sql script. This will replace the missing control on the Re-Create Document markup.
- Confirm that the script reports no errors, then type commit;
- Restart the MarkView Application Server in order for the change to take affect.
